COMPLEX EXPRESSIONS MOVEMENT STUDIO

ONLINE WAIVER & LIABILITY RELEASE

By enrolling myself or my child in classes, camps, workshops, intensives, rehearsals, performances, private lessons, or any activity offered by Complex Expressions Movement Studio, I acknowledge and agree to the following:

Assumption of Risk

I understand that dance, fitness, acrobatics, and related physical activities involve inherent risks, including but not limited to falls, collisions, strains, sprains, fractures, and other injuries. I voluntarily assume all risks associated with participation.

Medical Responsibility

I certify that the participant is physically able to participate in studio activities. I understand that it is my responsibility to inform the studio of any medical conditions, injuries, allergies, or limitations that may affect participation.

Release of Liability

I hereby release and hold harmless Complex Expressions Movement Studio, its owner, instructors, staff, volunteers, contractors, and affiliates from any and all claims, liabilities, damages, injuries, losses, or expenses arising from participation in studio activities, including classes, camps, workshops, performances, competitions, and events.

Emergency Medical Care

In the event of an emergency, I authorize Complex Expressions Movement Studio staff to seek emergency medical treatment for the participant if a parent or guardian cannot be reached immediately. Any medical expenses incurred are the responsibility of the participant or parent/guardian.

Photo & Video Release

I grant permission for Complex Expressions Movement Studio to photograph and/or record the participant during classes, rehearsals, performances, workshops, camps, and studio events. I understand that these images and recordings may be used for promotional, educational, social media, website, and marketing purposes without compensation.

Studio Policies

I acknowledge that I have reviewed and agree to follow all studio policies, procedures, and expectations. I understand that all tuition, registration fees, camps, workshops, intensives, private lessons, and special event payments are non-refundable and non-transferable.

Agreement

By checking the acknowledgment box and submitting my registration, I confirm that I have read, understand, and agree to the terms of this Waiver, Release of Liability, and Studio Participation Agreement.
